In consideration of being accepted as a participant in this workshop, the applicant 


and/or parents/guardians of the participant, individually named on this form, do 


hereby release PageantKnowledge.Biz, CP&G Services LLC, its staff, directors, 


instructors, subsidiaries, parent companies, assigns, employees, affiliates, 


licensees and servants, from any and all claims and damages directly or indirectly 


resulting from named participant participating in these workshop events; and I/we 


give permission to PageantKnowledge.Biz, CP&G Services LLC to use the participants 


photographs, speeches, testimonials, videos, etc., for publicity I/we hereby give 


PageantKnowledge.Biz,  CP&G Services LLC and licensees thereof the absolute right 


and permission to copyright and/or publish or use photographic portraits, pictures 


or videos of participant or in which participant may be included in whole or in 


part or composite or distorted in character or reproductions thereof in color or 


otherwise made through any media at any PageantKnowledge.Biz workshops or 


elsewhere for art, advertising, trade, or any other lawful purpose whatsoever, 


also to use participant name and photographs/film for all media, commercial film, 


television, video taping or promotional purposes at no compensation to participant.


I/we hereby waive participants right that I/we may have to inspect and/or approve 


the finished product of the advertising copy that may be connection therewith or 


the use to which it may be applied. I/we hereby release and discharge and agree 


to save PageantKnowledge.Biz, CP&G Services LLC and all subsidiaries, affiliates 


and licensees thereof, and their employees and representatives harmless from any 


liability by virtue of any blurring, distortion, alteration, optical illusion, 


or use in composite form whether intentional or otherwise that may occur or be 


produced in the taking of said pictures or in any processing tending toward the 


completion of the finished product. I/we hereby release PageantKnowledge.Biz, 


CP&G Services LLC, and all subsidiaries, affiliates, and licensees thereof, 


from all claims, demands, liabilities, and obligations whatsoever arising out 


of the use of such portraits or pictures by PageantKnowledge.Biz, 


CP&G Services LLC and subsidiaries, affiliates and licensees thereof.

application, which is applied to total class cost.  


Complete payment by workshop date is required for participation.


A full payment with a signed application earns an additional FREE CHANCE
